Back-End Accuracy

Payment posting

Posting is treated as clerical work in a lot of practices, which is exactly why it costs them money. Every downstream decision — what to bill the patient, what to appeal, what to write off, what your A/R actually is — depends on payments being posted accurately and read properly.

What payment posting covers

01

Electronic remittance posting

ERA files are posted with the correct allowed amount, paid amount, contractual adjustment, patient responsibility split and remark codes carried through to the account. Auto-posting is only as good as its exception handling, so exceptions are worked rather than left in a suspense file.

02

Paper EOB and correspondence

Paper remittances, offsets, recoupments and payer correspondence are posted and reconciled alongside electronic remittance so the ledger reflects everything the payer has done — not only the parts that arrived electronically.

03

Patient payments

Where in scope, point-of-service collections, statement payments and plan payments are posted and applied so patient balances stay accurate and statements don't chase amounts that were already paid.

04

Deposit reconciliation

Posted totals are reconciled against bank deposits and payer remittance totals. Unreconciled variances are investigated, not absorbed — an unbalanced day is a signal, and it stays open until it's explained.

05

Adjustment discipline

Contractual adjustments, administrative write-offs and denial-driven zero payments are recorded distinctly. When everything is posted as a contractual, denials disappear from your data and so does your ability to work them.

06

Discrepancy identification

Payments are reviewed against expected reimbursement. Underpayments, incorrect allowed amounts, unexpected patient-responsibility shifts and reprocessing errors are flagged for follow-up instead of being posted and forgotten.

Why posting quality shows up everywhere else

Posting is the point at which payer behaviour becomes data. Get it right and your denial reports, A/R aging and payer performance analysis are trustworthy. Get it wrong and every report built on top of it is quietly false.

A denial posted as a contractual adjustment never enters the denial worklist. An underpayment posted at face value never gets appealed. A recoupment applied without explanation turns into an A/R mystery three months later. None of these announce themselves — they just make the numbers wrong.

What accurate posting supports

  • Account balances that reflect what is actually owed, by whom
  • Denial data complete enough to work and to analyse
  • Patient statements that don't generate avoidable phone calls
  • Underpayments visible while they're still appealable
  • A/R aging that can be trusted for prioritisation
  • Clean reconciliation between the practice ledger and the bank
